From what I could find, this seems to be the most likely case: The signature inside the front cover appears to read "D F Jensen" or possibly "H F Jensen" written in an elegant, turn-of-the-century cursive script. Given the musical notes found inside, this strongly aligns with Donald F. Jensen or another classical musician/vocal coach of that era.
